This statement explains, in plain language, how Susan A. Baker, M.D., Inc. treats patient health information and how that information relates to our website at https://susanbakermd.com. It is not a HIPAA Notice of Privacy Practices. Patients receive our full privacy practices, and any required acknowledgements, directly at the office.

We are committed to protecting the privacy and confidentiality of our patients’ personal health information. This statement outlines, at a high level, how we collect, use, and safeguard that information, and clarifies that our website is not used to collect it.

2. What We Mean by Personal Health Information (PHI)

Personal Health Information (PHI) is identifying information about an individual relating to their physical or mental health, medical history, the health care provided to them, payment for that care, and associated health identifiers.

3. The Website Does Not Collect PHI

Our website is informational. We do not collect, store, or transmit patient health information through https://susanbakermd.com. Submitting information through the website does not create a patient-provider relationship, and patients should not send medical details through it. The website’s general data practices (such as analytics and cookies) are described in our separate Website Privacy Policy.

4. How PHI Is Collected at the Practice

We collect patient information directly from patients, or from authorized health care practitioners, in the course of providing care at the office. The purpose is to establish and support the patient-provider relationship and to meet patients’ medical needs. The handling of this information is addressed through the agreements and privacy practices patients receive at the office. We limit the information we collect to what is necessary for these purposes.

Consent is central to how we use and disclose PHI. Implied consent covers most routine health care purposes that follow from a patient’s consent to treatment. Other uses or disclosures require the patient’s express written consent, as described in the privacy practices provided at the office.

All staff members are responsible for protecting patient information, and we provide regular training so that staff stay current on our privacy practices.

7. Safeguards

We maintain physical, electronic, and procedural safeguards to protect patient information, and access to PHI is restricted to authorized personnel.
